To the Editor The Research Letter by Ms Hoffman and colleagues described trends in payer mix among patients receiving dialysis in the United States. We share the authors’ concerns that shifting patients from publicly funded insurance to commercial insurance could increase the overall cost of dialysis care. Although the Affordable Care Act increased access to the commercial insurance market through the creation of health insurance exchanges, we were surprised by the magnitude of non-Medicare coverage reported.
