We thank Marleen Hendriks and colleagues and Adam Richards for their comments regarding the HOPE 4 trial.1 Hendriks and colleagues draw attention to the generalisability of our findings as the non-physician health workers were paid by the project and the study medications were provided to the intervention group free of charge. Although working within the infrastructure of existing health-care systems might be theoretically optimal for uptake and sustainability, most health systems do not adequately address cardiovascular disease control.

However, the reduction in risk attributable to the care model is almost certainly larger than the ‚àí6 ∑40% points reported for the primary outcome for two reasons.

[Seminar] Atopic dermatitis
Sinéad M Langan, Alan D Irvine, Stephan Weidinger | thelancet.com | 2020-08-01
Atopic dermatitis is a common inflammatory skin disorder characterised by recurrent eczematous lesions and intense itch. The disorder affects people of all ages and ethnicities, has a substantial psychosocial impact on patients and relatives, and is the leading cause of the global burden from skin disease. Atopic dermatitis is associated with increased risk of multiple comorbidities, including food allergy, asthma, allergic rhinitis, and mental health disorders. The pathophysiology is complex and involves a strong genetic predisposition, epidermal dysfunction, and T-cell driven inflammation.

With great interest we read the encouraging results of the HOPE 4 trial by Jon-David Schwalm and colleagues.1 However, we agree with Tanzeen H Jafar and colleagues2 that the generalisability of the findings is limited because the non-physician health workers were privately paid by the study, and drugs were provided for free. Our work in Kenya shows that non-physician health workers are often overburdened, and that drugs are frequently out of stock in public facilities and pricey in private health facilities, leading to high out-of-pocket expenditure.

Lucy C Chappell and colleagues1 showed that, at the expense of increased neonatal unit admissions related to premature births, immediate planned delivery reduced maternal morbidity and severe hypertension compared with expectant management among women with late preterm pre-eclampsia. Although the authors conclude that their trial supports this strategy in favour of initiation of delivery in women with late preterm pre-eclampsia, we are concerned about the underestimation of long-term neonatal morbidities.

[Editorial] The truth is out there, somewhere
The Lancet | thelancet.com | 2020-08-01
The tidal wave of information on the internet concerning the COVID-19 pandemic has resulted in difficulties in discerning truth from fiction. This so-called infodemic, defined by WHO as an "overabundance of information–some accurate and some not–that makes it harder for people to find trustworthy sources and reliable guidance when needed", has become a major threat to public health. Infection rates will rise if people are confused about restrictions and patients may be harmed if they use unproven treatments or bogus remedies.

Infectious diseases have long shaped human history, from the plague of Athens (430–427 BCE) that killed around a third of the Athenian population, the Black Death in the 14th century that killed about 30–60% of all Europeans, and the 1918–19 influenza pandemic that took the lives of at least 50 million people globally. In the 8 months since the emergence of SARS-CoV-2, more than 600‚Äà000 people worldwide have died from COVID-19 as of July 21, 2020. [Comment] Screening for Barrett's oesophagus: is now the time?
Yuri Hanada, Kenneth K Wang | thelancet.com | 2020-08-01
Screening for cancer is an important aspect of medical practice in this new millennium. It is well accepted that to decrease mortality due to oesophageal carcinoma, it is necessary to identify the malignancy at earlier, curable stages. In The Lancet, Rebecca Fitzgerald and colleagues1 report the results of a prospective, multicentre, pragmatic, randomised controlled trial, done at 109 sociodemographically diverse general practice clinics in England, which investigated whether offering the Cytosponge-trefoil factor 3 (TFF3) procedure to patients on medication for gastro-oesophageal reflux would increase the detect…

UK government advisers issue stark warning: Avoid local lockdowns or unleash total anarchy
rt.com | 2020-08-01
As Britain imposes local lockdowns, a newly released study suggests the UK is a hair's breadth away from devastating riots. However, the government was warned about this a long time ago, and apparently chose to proceed anyway. | When the British government's policing and security advisers met in London on July 2, their tone was serious. Racial and societal tensions, they warned, had reached boiling point in the UK, and the re-imposition of coronavirus lockdown measures could ignite this powder keg and trigger riots unseen since the mass unrest of 2011. | If this happens, the advisers warned, police would be un…

Britain's Labour party is dishing out "substantial damages" to staffers who accused the party of anti-Semitism. 170th Anniversary of California and Negro Bar history still hidden?
Khubaka, Michael Harris | indybay.org | 2020-08-01
I've read lots of articles about Negro Bar and changing the name. But with our close tie to Mexico in the history of California, no one has mentioned that Negro means black in Spanish! | Are we also going to have to change the Spanish language? | So many of our towns and cities have some form of Spanish in their naming. They either start with San or Santa or the whole name is a Spanish word. So I rest my case. | Please bring this up in the next article about the name Negro being black in Spanish. | Laurie Kendall, Folsom…

When Corporate Power Is Your Real Government, Corporate Media Is State Media

Caitlin JOHNSTONE | astonishingly horrible article the other day titled "Latin America Is Facing a 'Decline of Democracy' Under the Pandemic" accusing governments like Venezuela and Nicaragua of exploiting Covid-19 to quash opposition and oppress democracy. | The article sources its jarringly propagandistic claims in multiple US government-funded narrative management operations like the Wilson Center and the National Endow…

Moncada's Significance in Cuban Revolutionary History
_____ | strategic-culture.org | 2020-08-01
"When Batista's coup took place in 1952, I'd already formulated a plan for the future. I decided to launch a revolutionary programme and organise a popular uprising. From that moment on, I had a clear idea of the struggle ahead and of the fundamental revolutionary ideas behind it, the ideas that are in History Will Absolve Me." | Just one year later, Fidel Castro would lead the attack on the Moncada Barracks in Santiago de Cuba on July 26, 1953. It was a failed military attempt and 70…

Forty five years ago, Cold Warriors in the Pentagon and CIA shook their fists angrily at the stars- and for good reason. | On July 17, 1975 the first international handshake was occurring in space between Russian Cosmonaut Alexei Leonov and American astronaut Thomas Stafford as the first official act kicking off the historic Apollo-Soyuz cooperative mission. To Reopen Schools Safely, We Need to Fund Teachers — Not Cops
Staff | truthout.org | 2020-07-31
Ask Randi Weingarten, president of the 1.7 million-member American Federation of Teachers (AFT), what public schools need in order to safely reopen this fall and her answer is immediate: more money for the daily deep-cleaning of schools; personal protective equipment (PPE) for teachers, staff and students; the hiring of thousands of additional school nurses and counselors to help students navigate social and emotional roadblocks; reliable computers and internet access for every kid; and a 10 percent increase i…

Here's How We Remake The Economy
_____ | popularresistance.org | 2020-07-31
To defend the $600 unemployment payments as both morally and economically necessary is not to suggest that the CARES Act is a sufficient solution to our economic problems. Nor, says Pavlina Tcherneva, Associate Professor of Economics at Bard College and a Research Scholar at the Levy Economics Institute and advisor to Bernie Sanders's 2016 presidential campaign, is the Democratic-led HEROES Act, which would extend the payments through January 2021 if Senate Republicans and the Trump administration would support it. Dr. Tcherneva studies the impact of unemployment on growth, income inequality and public health, an…

Civil Rights Hero John Lewis Remembered at Funeral as "Patriot Who Risked His Life" for Democracy
Staff | democracynow.org | 2020-07-31
As family, friends and dignitaries paid their final respects at the Atlanta funeral of John Lewis, the civil rights leader and 17-term Georgia Congressmember was remembered as a singular force for equality and justice. The funeral took place at the historic Ebenezer Baptist Church, once led by Dr. Martin Luther King Jr., where senior pastor Rev. Raphael Warnock contrasted Lewis' legacy with "some in high office who are much better at division than vision," and described the late politician as "a true American patriot who risked his life and bled for the hope and promise of democracy."

Barack Obama: Honor John Lewis by Renewing Voting Rights Act & Ballot Access in the U.S
Staff | democracynow.org | 2020-07-31
In his stirring eulogy at the funeral service for Congressmember John Lewis, President Barack Obama said expanded voting rights would be the greatest way to honor the civil rights icon's legacy. In a speech that condemned the status of American democracy without ever naming the sitting president, Obama called for election day to be declared a national holiday, full Congressional representation for Washington, D.C., and Puerto Rico, and the end of the filibuster, which he called a "Jim Crow relic." "You want to honor John? Let's honor him by revitalizing the law he was willing to die for," Obama said in reference…
