[Perspectives] Caring intensively: a GP’s pandemic journal
Rachel Clarke | thelancet.com | 2021-01-30
“What do we live for, if it is not to make life less difficult to each other?” asks Dorothea Brooke, heroine of one of the greatest Victorian novels, George Eliot’s Middlemarch. Her question is simultaneously mundane and profound. It conjures all those discreet and humble acts–a hug, an offer of shelter, a door held open to allow a grandparent to pass–that when knitted together help people feel cared for and make society humane. One senses that, like Dorothea Brooke, author and Edinburgh general practitioner (GP) Gavin Francis would struggle to see the point of a life lived without caring for others.

[Seminar] Multiple myeloma
Niels W C J van de Donk, Charlotte Pawlyn, Kwee L Yong | thelancet.com | 2021-01-30
Multiple myeloma is the second most common haematological malignancy in high-income countries, and typically starts as asymptomatic precursor conditions–either monoclonal gammopathy of undetermined significance or smouldering multiple myeloma–in which initiating genetic abnormalities, such as hyperdiploidy and translocations involving the immunoglobulin heavy chain, are already present. The introduction of immunomodulatory drugs, proteasome inhibitors, and CD38-targeting antibodies has extended survival, but ultimately the majority of patients will die from their disease, and some from treatment-relat…

Manipulation was the most cost-effective option.

In the SENIOR-NSTEMI study, Amit Kaura and colleagues1 exploit state-of-the-art statistical approaches including target trial design, propensity score-adjusted regression, and inverse probability of treatment weighting, with the results clearly favouring an invasive management strategy in older patients (‚â•80 years old) with NSTEMI. However, some additional details should be provided by the investigators, to avoid any suspicion of residual confounding factors and strengthen the evidence that the invasive strategy is actually and mechanistically beneficial.

[Seminar] Biliary tract cancer
Juan W Valle, R Katie Kelley, Bruno Nervi, Do-Youn Oh, Andrew X Zhu | thelancet.com | 2021-01-30
Biliary tract cancers, including intrahepatic, perihilar, and distal cholangiocarcinoma as well as gallbladder cancer, are low-incidence malignancies in most high-income countries, but represent a major health problem in endemic areas; moreover, the incidence of intrahepatic cholangiocarcinoma is rising globally. Surgery is the cornerstone of cure; the optimal approach depends on the anatomical site of the primary tumour and the best outcomes are achieved through management by specialist multidisciplinary teams.

We thank Georgios Mamarelis and Dimitrios Moris for their interest in the UK FROST trial data1. As we reported,1 hydrodilatation was being used in only a few centres when we designed the UK FROST trial, which meant that it was not feasible to include hydrodilatation as a trial intervention. Although we acknowledge that hydrodilatation seems to be used more widely nowadays, the evidence to support its use remains scarce. There is also uncertainty as to what constitutes hydrodilatation because there is considerable variation in how it is delivered.

Low-income and middle-income countries (LMICs) have a large burden of cancer mortality compared with high-income countries, with as many as 70% of global cancer deaths in these regions and a paucity of data to drive cancer policies.1,2 Several factors contribute to this burden, ranging from late presentation of disease to poor access to diagnosis and treatment.2 Access to surgical interventions is reduced in LMICs, and the risk of complications of surgery is high in these regions.3–5 When surgical interventions are available, knowledge about how to avoid complications can be scant.

Biden Administration Halts Arms Deals Over Yemen… What’s Really Going On?
_____ | strategic-culture.org | 2021-01-30
The bigger concern of Biden and Blinken seems to be damage limitation to America’s bloody image, and also doing a big favor for Israel. | Antony Blinken, the newly-appointed US top diplomat, has announced a pause in arms sales to Saudi Arabia and the United Arab Emirates out of apparent concern over American weapons being used in prosecuting the horrendous war in Yemen. | Blinken was confirmed by the Senate for…

‘French Revolution In Finance’
_____ | popularresistance.org | 2021-01-30
The New York Stock Exchange suffered a poor day on Wednesday, with the Dow Jones Industrial Average sinking by more than 600 points. However, for a select few companies, stock values have gone through the roof. | It was thanks, in large part, to a group of amateur stock traders with a vendetta against veteran corporate insider short-sellers betting on a company’s failure, with the video game store GameStop taking center stage. Before the drama was over, the company’s value had jumped by more than 1,700%, an investment firm had lost billions, and some small-time investors who gambled it all were able to afford som…

We Should All Be Outraged
_____ | popularresistance.org | 2021-01-30
Someday the world will be free of the coronavirus. Then, we will glance backward at these years of misery inflicted by virions with spike proteins that have struck down millions of people and held social life in its grip. Much will be debated about the origins of the virus and the rapidity of its spread around the world, a transmission that shows how closely interconnected we have become due to modern transportation technology. There is no going backward from the processes that will continue to shrink the globe, bringing us closer and closer together, bringing other viruses and diseases greater and greater host p…

How The Wet’suwet’en Solidarity Actions Changed Their Lives
_____ | popularresistance.org | 2021-01-30
It was the first week of Kolin Sutherland-Wilson’s final semester at the University of Victoria. But he wasn’t there. Instead, on a chilly January morning in 2020, he sat alone on the front steps of the British Columbia legislature, dressed warmly and holding signs that called on provincial leaders to stand with the Wet’suwet’en Hereditary Chiefs opposing the Coastal GasLink project in their traditional territory. | For a week, he spent all day on the steps. MLAs and staff who passed by barely glanced at him. | But soon friends, classmates and community members joined him. The growing group took on bigger actions…

The Trade Union Movement Must Organise Minimum Wage Workers
_____ | popularresistance.org | 2021-01-30
The twenty-first century has seen the Trade Union Movement in Trinidad and Tobago consistently under attack, severely criticized and victimized by the ruling economic and political elites. | The thousands of sugar workers were the first group of organised workers this century to suffer mass retrenchment. This, of course, has had the effect of severely weakening a once powerful union. | The Unions in petroleum and petrochemical industries have seen a steady decline in their workforce Thousands of direct and indirect Petrotrin workers have been thrown on the breadline as have hundreds of workers at Arcelor Mittal a…

UN Independent Expert: ‘This Is How The Human Rights Industry Works’
_____ | popularresistance.org | 2021-01-30
Indeed, a “human rights industry” exists, and many officials of the UN’s Office of the High Commissioner [for Human Rights], diplomats, activists from non-governmental organisations, human rights teachers, and, of course, also many “independent experts” and rapporteurs are more or less opportunistic. | These people consider themselves “better” than others, but do not necessarily feel solidarity with the victims of human rights violations. Some of my bosses were models of integrity and professionalism, such as Professor Theo van Boven, Professor Kurt Herndl, Judge Jakob Muller, or High Commissioners Bertrand Ramch…

Snipers, Landmines, Death: Life On Yemen’s Frontline
_____ | popularresistance.org | 2021-01-30
Almost every building is damaged. Dogs bark at anyone walking down the empty streets, and in the distance the sound of children playing in alleyways is interrupted by occasional gunfire. | Carpets are hung between buildings – not to dry, but to protect passersby from snipers. | These are the frontlines of Taiz, a war-wracked city in southern Yemen where terror follows its long-suffering residents. | Most Yemenis have fled the homes that now lie between the pro-government forces and Houthi fighters. But some remain – either refusing to abandon their houses and live in displacement, or simply lacking any alternativ…

How A US-Backed Coup In Serbia Inspired The DC Insurrection
_____ | popularresistance.org | 2021-01-30
As the Capitol insurrection on January 6 descended into a frenzy of mob violence, a far-right paramilitary group known as the Oath Keepers marched through the sea of rioters and protesters, up the east steps of the Capitol building and into the rotunda. Video from that day shows the Oath Keepers marching in a military formation known as “Ranger File”, described by the Associated Press as ‘standard operating procedure for a combat team that is “stacking up” to breach a building — instantly recognizable to any U.S. soldier or Marine who served in Iraq and Afghanistan.’ | On November 10, 2020, nearly two month…

ACLU Pressing Biden To Stick To His Promise Of Decarceration
_____ | popularresistance.org | 2021-01-30
The American Civil Liberties Union (ACLU) is launching a six-figure ad buy featuring both digital and print spots calling for President Biden to adhere to his campaign promise of decreasing the number of incarcerated persons in the country. | Specifically, the ACLU wants Biden to grant clemency to thousands of people who meet certain criteria, something that he could do through executive powers. | “We use criteria that reflect an evolution in our thinking around criminal justice,” Cynthia Roseberry, deputy director of the ACLU’s justice division, told The Hill. | “For example, calling for those who are now incarc…

The far-right attempts to seize opening from the 1619 Project
WSWS repost | indybay.org | 2021-01-30
The authors of the 1619 Project and the 1776 Report have more in common than they may care to admit. Both replace class struggle in history with forms of nationalist mythmaking–right-wing black nationalism in the former, and far-right “America First” nationalism in the latter. Both falsify America’s revolutionary history in service of the present-day political needs of factions of the ruling class–the 1619 Project for the Democratic Party and its satellites; the 1776 Report for Trump and the Republicans. Neither the 1619 Project nor the 1776 Report can tell much of the actual history of the United Sta…
